Medications You Don't Want to Take with Food - And Those You Do



Food has varying effects on your medications. They can Reduce how much of the medicine is absorbed, they can INCREASE how much is absorbed or they can delay when the medication is absorbed.

Reduced absorption means you don't get as much of the medication into your bloodstream and therefore it may not work as well as expected. Increased absorption means that you can get more side effects once the medication reaches its peak concentration in your bloodstream. Delayed absorption means that it takes longer to have the desired effect. This becomes most important with the sleeping medications

Adding to pandemic complications, local restaurants are struggling as a boil water advisory enters its fifth day

Several months into the coronavirus pandemic, restaurants in Lake Jackson, Texas, have a new health challenge to deal with: a boil-water advisory that started last week and could last for several days.

Late on Friday night, September 25, Brazoria County officials issued a Do Not Use water advisory that covered nine Gulf Coast Texas cities, including Lake Jackson, Freeport, Angleton, and Rosenberg. The advisory came after the death of a 6-year-old boy, attributed to a rare infection of the organism Naegleria fowleri. N. fowleri, often referred to as the “brain-eating amoeba,” is a naturally occurring organism found in fresh, warm water. It typically enters the human body through the nose, where it then travels to the brain. Infections are rare — there were only 34 reported cases in the United States from 2008 to 2018 — but often deadly.

By Saturday morning, officials had narrowed down the problem to the city of Lake Jackson’s water supply. (Officials believe the boy may have been infected from a water hose at his home, or at a local park’s splash pad.) The Do Not Use advisory was downgraded to a boil advisory for Lake Jackson only. Residents have been lining up for free cases of bottled water at the local college. On Monday, three days after the crisis began, Texas Gov. Greg Abbott issued a disaster declaration.

For restaurants in the city, the water crisis is yet another challenge on top of the COVID-19 pandemic. Audra Robinson owns three restaurants in Brazoria County — Asiel’s, in the city of Clute; Poly Pop, in Lake Jackson; and Dirty South, in Angleton. On Friday night, all three restaurants were under the Do Not Use advisory. Before the advisory was downgraded, Robinson made the decision to keep the popular Asiel’s closed for Saturday breakfast and brunch.

“It was devastating,” she says. “We make as much money for Saturday breakfast as we do all day Monday. Plus, people are still afraid, so it was a slow weekend.”

As of now, only Poly Pop, Robinson’s tiki bar in Downtown Lake Jackson, remains under the boil advisory. It reopened on Sunday. And Robinson is somewhat lucky — even though Poly Pop and Asiel’s are in different municipalities — and thus use different water sources — the restaurants are only about half a mile from each other. So Robinson’s staff have been toting gallons of fresh water to Poly Pop for use every day, as well as things like pre-brewed ice tea and other staples.

Still, as a cocktail bar, Poly Pop has unique needs. Staff there cannot use the soda machines right now, so Robinson is buying bottled sodas from her distributor. She’s also buying crushed ice for the cocktails from the local Sonic. All those extra costs are adding up, she says, and some of her neighboring restaurants aren’t able to handle the extra precautions.

“Chili’s is still closed,” she says. “The bigger you are, the less agile.”

For now, the boil advisory is still in effect. According to the Texas Commission on Environmental Quality, city officials are actively working on a plan to flush and disinfect the water system, and the advisory won’t be lifted until the water consistently tests negative for the amoeba. It’s unclear how long that could take, but Robinson says she’s heard rumors that it could be as long as 60 days. And she’s annoyed at the government’s response. Pallets of water aren’t enough, she says. But by now, in the eighth month of the pandemic, her staff is used to rolling with the punches.

“My staff are always on social media, checking the news and the TCEQ accounts,” she says. “We’re generally more informed than the city. This is just like the virus — it’s such a fast-moving situation, with a lot of confusion and misinformation.

“It’s been the same process as the virus — freak out, shut down, reopen slowly.”

Having previously pooh-poohed the dangers of the coronavirus, Richard Epstein is now calling for an immediate end to social distancing and business shutdowns.  It’s unfortunate that he has chosen to dig himself deeper rather than admitting his mistake and moving on.

As I discussed in a previous post, Epstein initially predicted that the coronavirus would cause about 500 deaths in the U.S., which he then revised to 5000.  The U.S. is now at 50,000, which is what Epstein initially predicted as global mortality.

Epstein says that his estimates are more accurate than initial estimates of millions of U.S. deaths (perhaps referring to the two million deaths estimated by Imperial College, London, a widely publicized forecast).  But that estimate explicitly assumed no government intervention.  Epstein’s estimate of the number of deaths  without government intervention was one-tenth the actual number that happened despite vigorous government intervention. That isn’t saying much for his powers of prediction.

In mid-March, Epstein said confidently that “it is highly unlikely, given the relatively short (two-week) incubation period, that the number of current cases will more than double or triple. ” On March 15, there had been 3600 U.S. cases and 121 deaths. Now we’re at 850,000 cases and 50,000 deaths.  His prediction was flawed.  This might cause you to doubt how much faith you should put in his views of epidemiology.

Epstein’s confidence in his powers as an epidemiologist has only continued to grow.  In a recent interview with Vox, he said: “If I’m correct in what I said, the dominant model has about a one percent chance of being true, and my model, forget about the differences, is essentially you’re talking about normal flu season, 30,000 to 50,000 deaths, is 99 percent true.”

Epstein continues to defend his estimates.  He insists that the reported numbers are much too high. Indeed, he says, “the evidence is overwhelming that the virus by itself kills few individuals.”

Epstein’s dispute with the reported figures relates mostly to preexisting conditions. He argues that most coronavirus victims should not be counted because preexisting conditions contributed to their deaths. I’m a bit surprised that an expert in tort law would take that position.  Yes, people had preexisting conditions. The fact remains they would be alive today but for the coronavirus.   As we all tell students, something can be a “but for” cause of harm even though other contributing factors were also necessary to the outcome.

Think of it this way.  Suppose some terrorist lets loose a virus that targets people who suffer from obesity, heart trouble, or diabetes, resulting in ten thousand deaths.  Epstein’s view apparently is that, if we’re counting annual deaths from terrorism, we should deduct some or all of these victims from the count.

Epstein makes two other arguments.  The first is that the number of new infections is declining, so we no longer need social distancing.  This is like saying that you should stop chemotherapy because it’s starting to shrink your tumor.

The other argument is that the government doesn’t need to do anything because the market will solve the problem.  Businesses will compete to demonstrate their safety and people will take their own precautions.  It’s certainly not surprising that a libertarian would embrace the magic of the market as a solution to most any problem.

Any economist could shoot a dozen holes in Epstein’s reliance on the market in this situation.  In fact, law and economics has had so much impact in law schools that most law professors can see the flaws. Feel free to skip this next part, however, if you find the specifics a bit geeky.

If Epstein gave a faculty workshop on the subject, here are a few points he’d get hit with:

  1. People’s behavior will take into account the risks that they themselves face, but not the risk that they will infect others. Economists call this an externality, and markets don’t solve those.
  2. A related externality is that greater risk-taking by some people will require additional costly precautions by those who are at risk. Another externality.
  3. There’s also what economists call a congestion externality.  Drivers don’t take into account that their morning drive to work will slow down other traffic.  Similarly, people won’t take into account the harm they may do others simply by clogging the medical system, reducing medical care available to others.
  4. People have imperfect information about what precautions businesses are actually taking. Imperfect information also screws up markets. If people can’t be sure of telling a safe business from an unsafe one, there’s no competitive advantage in being safe.
  5. Businesses  have imperfect information about what their employees are doing, which may encourage shirking by employees regarding safety measures. So businesses may not be able to deliver on promises of safety.
  6. Individuals are risk averse, but insurance markets may not be adequate to spread risks, leading them to take excessive precautions to avoid the risks.

In other words, this is a situation rife with market failures.  The market will no more be able to solve this problem than a sieve will work as a life preserver.

I don’t want to argue the general merits of libertarianism.  Maybe, although I’m not convinced of this, it’s actually the right answer much of the time. Even so, a pandemic is not one of those times.

Epstein is right that we can’t be sure what will happen if we relax social distancing at a point where the infection is slowing.  There’s a lot we still don’t know about this virus, and we can’t be sure of how businesses and people would respond. Maybe things wouldn’t get as bad as experts now predict.  But what Epstein overlooks is that uncertainty cuts both ways: the outcome could also be much worse than the experts predict.

Epstein’s basic mistake is common among really bright people. It’s a tendency to overestimate their ability to make expert judgments outside of their own domain.  It comes along with a tendency to assume that people in other fields are pretty dumb – for instance, that epidemiologists are unaware of the risks of shutting down elective medical services.

It’s a free country, and Epstein is entitled to his views.  What’s worrisome is that the Trump Administration has a track record of listening to him.

A popular Korean street food restaurant is bringing its crispy, stuffed corn dogs to Bellaire Food Street, the popular food hall in Houston’s Chinatown, this fall.

Myungrand Hot Dog is known for its deep-fried hot dogs on a stick, stuffed with all manner of creative fillings, like gooey mozzarella, squid ink, crispy potatoes and more. It’s been called the most famous hot dog in Korea. The restaurant has more than 700 locations in Korea, and has opened 15 locations in the United States since they first launched here in 2018, including one in Carrollton.

In Houston, Myugrand will rebrand as Two Hands, located at 9393 Bellaire Street. A Two Hands also opened in Lewisville, Texas, in January. A representative for Bellaire Food Street explained that it’s the exact same restaurant — Two Hands is just the new name for the English-language market.

When it opens, Two Hands will join Sul Bing Su in Katy, which also serves Korean-style hot dogs alongside decadent shaved ice. In addition, Bellaire Food Street already includes a bevy of popular street foods, including Korean fried chicken, spicy hot pots, and fancy popsicles.

No word yet on an exact opening date, but stay tuned for details.

Georgia has two Senate contests due to a fluke of timing — one a regular election, the other a special election.  The regular election pits David Perdue (R) against Jonathan Ossoff (D). The special election pits Kelley Loeffler (R) against an open field, where her principal Democratic opponent is probably Rev. Raphael Warnock, the African-American pastor of the historic Ebenezer Baptist Church where Martin Luther King was once pastor. The Republican candidates are favored in both races, but they aren’t necessarily out of reach for the Democrats.

The Regular Election: Perdue versus Ossoff.

David Perdue.

Perdue has a 3% lifetime score from the LCV, fairly standard for a conservative Republican. His Senate website doesn’t have an Issues tab, but it does have one for “Donuts with David.” Which admittedly does sound like more fun.

Jonathan Ossoff.

Ossoff’s campaign website says, “The scientific consensus is unambiguous: if pollution from fossil fuel combustion is not controlled, the consequences will be dire.” He pledges to “suppport a historic infrastructure plan that includes massive investments in clean energy, energy efficiency, and environmental protection,” as well as pushing for the U.S. to rejoin the Paris Agreement and for EPA to reverse Trump’s rollbacks. Ossoff is endorsed by the League of Conservation Voters.

The Special Election: Loeffler versus the Field

Kelley Loeffler.

One of Loeffler’s problems is that the Republican vote is likely to be split. Doug Collins, a GOP member of the House, is also running. Collins has a lifetime LCV score of 3%, making him a standard-model anti-environmental Republican House member. It’s possible that only the two Republicans will advance to the runoff. But it’s also possible that the Democratic challenger Warnock will actually finish #1 in the primary.

Raphael Warnock. Warnock’s website is pretty barebones. He pledges to “focus on fighting for quality, affordable health care, for the dignity of working people who are paid too little as our government works more for Wall Street, and to make sure every voice is heard.” He has a history of bringing environmental issues into his work as a pastor. He told LCV, “Too often, fossil fuel lobbyists and politicians have taken advantage of the revolving door between corporate boardrooms and political backrooms so much that we cannot tell the difference between the two.”

It seems very unlikely that any candidate can win a majority on Nov. 3, which means that top two will get into a runoff. The crucial question at this point is whether Democrats can unite around Warnock or some other candidate, which should get them into runoff.  Otherwise, the runoff will be between two Republicans, Loeffler and Collins.
